Checklist Before Surgery

Wear comfortable, loose fitting clothes

Leave all jewelry and valuables at home

Bring cases for eyeglasses, contacts, or dentures

Bring insurance identification cards, driver's license, co-payments, etc.

Bring all medications

Make sure you have a ride home, and someone to take care of you when you get home

Bring a bottle or "sippy cup" for your child

Remove any body piercing jewelry

Have all pre-registration forms filled out, or be ready to do so at the center
